About the Role
Responsible for coordinating and implementing system patient safety strategies and best practices locally, promoting a culture of safety and high reliability. This role involves developing, implementing, improving, and evaluating patient safety systems while partnering with Infection Prevention, Quality, Experience, Advocacy, Accreditation, Clinical Quality Data, and Risk.
Responsibilities
- Contributes to deploying the Pulse program to the organization and supports evidence-based practices including High Reliability, Just Culture, and Culture of Safety concepts.
- Develops and participates in patient safety orientation, educational endeavors, annual recurrent training, one-on-one consultations, and in-service training.
- Supports safety huddles and performs rounds to proactively identify safety risks and hazards, evaluating processes to ensure adherence to the Pulse program and standard work practices. Recommends enhancements for improving patient, team member, and environmental safety.
- Completes review of Pulse Power submissions to identify patterns or trends, facilitating opportunities for improvement and supporting efforts to prevent future events.
- Works in collaboration with leadership and team members to ensure timely reporting and management of patient safety issues and findings at the operational level.
- Provides and communicates patient safety trend reports to stakeholders and leaders, assisting in the development and monitoring of action plans.
- Serves as a partner on patient safety-related subjects, providing direct support to programs that involve patient safety.
- Participates in the development of administrative and clinical policies regarding patient safety.
- Works with stakeholders to implement programs designed to maximize patient safety performance and evaluation as established by the National Quality Forum (NQF) and The Joint Commission (TJC) National Patient Safety Goals (NPSG) and Sentinel Event Alerts.
- Conducts training and coaching, develops templates, tools, and processes to facilitate efficient and effective patient safety improvement.
- Disseminates patient safety best practices and educational materials, communicating evidence-based ideas throughout the organization.
- Facilitates thorough root cause analysis (RCA) through the RCA process following criteria for ECA and ACA, and facilitates development of strong action plans.
- Assists operational leaders with implementation of relevant strategies and disseminates lessons learned from the RCA process and recommendations across the organization.
- Prepares, provides, and participates in the reporting of patient safety-related information to Leadership, Governing Board, and other applicable committees.
- Participates in annually reviewing the effectiveness of the Patient Safety program and contributes to organizational goal setting and prioritization of initiatives.
- Partners with Infection Prevention, Quality, Experience, Advocacy, Accreditation, Clinical Quality Data, and Risk to integrate and assimilate information obtained from data analysis and operational issues related to risk.
- Analyzes findings and works collaboratively with key stakeholders to support the Pulse program and enhance an organizational culture that supports safety and reduces risk.
- Performs other duties as assigned.
Requirements
- Bachelor's Degree in Nursing or Healthcare-related field of study.
- Two (2) years of related experience; hospital/health care patient safety experience preferred.
- Certification required within two years of hire into role: Certified Professional In-Patient Safety (CPPS), Certified Professional in Healthcare Risk Management (CPHRM), Associate in Risk Management Certification (ARM), or Certified Professional in Healthcare Quality (CPHQ). Safety Partners in role as of June 1, 2023, have two years from that point to earn a certification.
- Valid driver's license and acceptable driving record (for Blount location).
