About the role
Rocky Mountain Senior Care is the largest post acute care practice in Colorado with 100+ physicians, nurse practitioners, and physician assistants covering 700+ Assisted Living facilities, independent living facilities, skilled nursing homes, memory care facilities and group homes stretching from Colorado Springs to Fort Collins with Denver in the middle. The practice philosophy is structured so you determine your work/lifestyle balance. With the 700+ facilities you are challenged by the wide spectrum of pathology that will test all of your clinical skills.
Responsibilities
- Provide comprehensive medical care to patients in a variety of settings including Assisted Living facilities, independent living facilities, skilled nursing homes, memory care facilities and group homes.
- Diagnose and treat a wide range of medical conditions, including those affecting older adults.
- Collaborate with other healthcare professionals to ensure patient care coordination.
- Manage patient care plans and coordinate with insurance providers.
- Participate in continuing education and professional development opportunities.
Requirements
- Geriatric or Senior Care Experience
Qualifications
- Master's degree in Nursing or Physician Assistant program
- Current license to practice as a Nurse Practitioner or Physician Assistant in Colorado
- Experience in outpatient Internal Medicine or Family Medicine
- Strong clinical skills
- Excellent communication and interpersonal skills
- Ability to manage multiple tasks and prioritize effectively
- Knowledge of geriatric care and senior health issues
