RNAC/MDS Coordinator

Key Responsibilities

• Conduct comprehensive interdisciplinary assessments in accordance with federal regulations
• Oversee the completion and accuracy of the Minimum Data Set (MDS) assessments
• Serve as a liaison between residents, families, and the interdisciplinary team to facilitate care planning
• Ensure documentation aligns with regulatory standards and internal policies
• Support care teams with accurate care plans and progress notes
• Maintain compliance with CMS, state, and local guidelines

Required Skills And Qualifications

• Current Registered Nurse (RN) licensure
• Minimum of two years’ experience with MDS and long-term care assessments
• Familiarity with the MDS 3.0 process and federal healthcare regulations
• Strong organizational skills and attention to detail
• Excellent communication and teamwork abilities
• Able to manage multiple priorities effectively

Preferred Skills And Experience

• Experience with restorative nursing programs
• Knowledge of compliance and audit procedures
• Prior experience working in a skilled nursing or senior living environment

Compensation

$80,000 - $100,000/yr + Sign on Bonus and Extensive, very generous benefit package!
