Primary location: B. Department: Vascular Access Team. Full-time night shift (36 hours/week, 0.9 FTE).
Responsibilities
- Apply clinical judgment by synthesizing and interpreting multiple data sources, anticipating problems, and seeking interprofessional collaboration to respond to complex situations.
- Lead efforts to improve documentation of the plan of care, care delivered, and outcomes of care.
- Engage patients and families in the plan of care, addressing current and future needs.
- Collaborate with families, healthcare providers, and community resources to develop comprehensive care plans.
- Orchestrate caring practices to address patient and family comfort and concerns, promoting a healing environment for all outcomes, including end-of-life care.
- Integrate age-specific and culturally inclusive concepts into patient care, considering both chronological age and developmental level.
- Develop care goals and actions in partnership with patients and families using self-management strategies.
- Modify or develop patient and family education programs creatively; evaluate effectiveness using teach-back methods and behavioral observation.
- Integrate patient and family education throughout care delivery and support transitions to home.
- Use systems thinking to navigate and negotiate care coordination on behalf of patients and families.
- Lead efforts to change systems and processes to support optimal patient outcomes.
- Maintain an understanding of regulatory requirements and accreditation standards; assist in meeting and sustaining compliance efforts.
- Monitor and ensure compliance with safety and departmental policies and procedures; fulfill individual certifications and Safety College requirements.
- Communicate safety and compliance concerns assertively to leadership.
- Coordinate team care delivery as charge nurse, anticipating and mitigating unexpected circumstances.
- Demonstrate integration of the Interprofessional Practice Model (IPM) in all aspects of practice.
- Advocate for patients, families, and communities from their perspective, empowering them to represent themselves and achieve mutuality in relationships.
- Use inclusive resources to enhance care experiences for diverse patient populations and incorporate differences, including alternative therapies.
- Grow self and others in cultural competence.
- Collaborate with teams using shared decision-making, promoting and expecting collaborative behavior.
- Mentor others to respect the contributions of the entire healthcare team.
- Lead in shared governance structures at cluster or divisional levels, including councils, committees, task forces, or projects.
- Improve, deviate from, or individualize standards and guidelines for specific patient situations or populations.
- Question and evaluate current practice based on patient responses, literature review, research, and education.
- Acquire knowledge and skills to address practice questions and improve patient care.
- Create practice changes through research utilization and experiential learning.
- Develop competence in peers through precepting and sharing knowledge at the point-of-care.
- Demonstrate awareness of unit competency needs and provide leadership to improve staff competence.
- Use self-appraisal, performance review, and peer review for competence assurance and professional development.
- Precept nursing peers and students; facilitate learning for healthcare team members and the community.
- Lead in identifying, sharing, implementing, and evaluating safety best practices with department managers and magnet representatives.
Requirements
- BSN from ACEN, CCNE, or NLN CNEA accredited institution or MSN required.
- 5+ years of related experience.
- Active Ohio RN License; may require additional state licensure.
- Active certification; some roles require specialty certification.
