About the Role

The RN serves as a consistent care coordinator throughout the continuum of care for patients entering or within our health system. Assesses the physical, psychological, and social needs of the patient, and improves the care experience for the patient and their family. Responsible for the nursing care of patients in a clinical or office setting. Coordinates communication with providers on behalf of the patient. Meets the needs of the patient in relation to education, screening, prevention, and follow-up care. Models appropriate behavior as exemplified in MLH Mission, Vision, and Values.

Responsibilities

  • Assesses the physical, psychological, and social needs of the patient, and improves the care experience for the patient and their family.
  • Creates a seamless flow through the health system to facilitate the patient’s care.
  • Develops and coordinates education, resources, and activities that provide understanding and support for patients and families.
  • Maintains reliable systems to document, track, and monitor patient data.
  • Performs clinical procedures within the scope of nursing practice and clinical training.
  • May perform telephone duties within the scope of the RN role.
  • Performs other job functions as assigned or requested.

Requirements

  • Required: Associate’s Degree in Nursing
  • Preferred: Bachelor’s Degree in Nursing
  • Required: Registered Nurse with 0-1 years of experience
  • Preferred: 1-3 years of clinical nursing experience
  • Required: Basic Life Support certification (American Heart Association)
  • Required: Registered Nurse license (Tennessee Board of Nursing)
  • Preferred: Certification in clinical specialty

Skills

  • Knowledgeable of system’s health resources to navigate patients to appropriate care.
  • Excellent verbal and written communication skills.
  • Strong problem-solving and conflict resolution abilities.
  • Proficient in computer skills.
  • Role model in customer service and patient care.
  • Displays sensitivity and compassion.

Physical Demands

  • Physical activities may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
  • Must have good balance and coordination.
  • Medium work: exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently.
  • Close visual acuity required, including color, depth perception, and field of vision.
  • Frequent invasive and non-invasive patient contact.
  • Exposure to patient body fluids, hazardous or poisonous materials.
  • Ability to react quickly to emergency situations.
  • Ability to handle and administer medications, vaccinations, and immunizations as needed.
