Responsibilities
Care Needs & Planning: Assist to develop, implement and continually update individual care plans, supplement hands on care needs
Assessments: Complete admission, discharge and other assessments as needed
Triage & Interventions: Assist to handle acute health changes, physicians' orders and collaboration with care providers & families
Regulatory Standards: Ensure compliance with federal, state, county & company health/safety regulations
Quality Assurance: Audit documentation & care for quality control/compliance
Family & Team Liaison: Communicates changes in resident conditions to families/providers, works collaboratively with the interdisciplinary team
Team Oversight & Mentorship: Provide ongoing training, orientation & performance evaluations for staff. Manage staffing schedules and assignments
Requirements
Required: Prior Long Term Care Experience
Qualifications
License to work within the State of Connecticut as a RN Registered Nurse
Able to pass background check, drug screen, professional references check & physical evaluation
High level understanding of leadership, customer service and clinical competency
Leadership experience in a long-term care setting
