Create personalize care plans assess patient needs and coordinate with other healthcare providers to ensure a seamless and effective transition with the goal of reducing Emergency Room readmission. These navigators would provide post-discharge follow-up education and care coordination via phone or digital platforms helping to bridge the gap between ED care and long-term management. Focus on cardiac conditions.
