About the role
Transform Healthcare Hospice Team Lead resolves department issues, aware of organizational developments and initiatives and implements as needed for their team. In addition to the lead responsibilities, maintains clinical competencies and provides professional care to patients of all ages in response to agency needs.
Responsibilities
- Affirms assessments, plans, implements, and evaluates patient care in order to achieve optimum patient outcomes for patients as appropriate to service area.
- Leads the team to achieve positive patient outcomes.
- Follows-up on matters to ensure they are successfully resolved.
- Aids leadership in assuring staff competence and contributes to staff performance development.
- Identifies areas of improvement, provides input, works with employees and follows up to ensure employees are on track for success.
- Maintains performance related information and is part of annual reviews.
- Conducts individual case conferences and participates in interdisciplinary team meetings to facilitate effective communications.
- Mentors and coaches employees; provides performance feedback.
- Listens to staff needs and concerns and provides resolutions.
- May follow up with staff members on identified issues and concerns and reports to leadership.
- Aids with coordinating workload for the home care/hospice team.
- Maintains adequate staffing levels, monitors work hours, and performs patient visits as necessary.
- Handles clinical issues for the agency and escalates to leadership as needed and apprising them of the situation.
- Maintains staff productivity overtime, mileage, and supply usage to ensure appropriate resource allocation and expense control.
- Collaborates with leadership on fiscal management.
- Aids with other agency responsibilities including but not limited to; implementing and managing daily activities of the office, maintaining regulatory compliance, reviewing documentation, ICD-10 coding, monitoring documents, re-certifications, performance improvement and other quality improvement initiatives.
- Ensures Joint Commission, Medicare and other third party payer guidelines are implemented and followed.
- Ensures compliance with policies and procedures.
- Promotes positive work environment by holding staff accountable while using effective interpersonal skills.
Requirements
- Education: Graduate of accredited Registered Nurse program.
- Experience: 5 years of clinical experience required.
- Licensure, certification, and/or registration: Currently licensed as Registered Nurse in the state of Virginia or approved compact state.
- Must have valid driver's license, automobile insurance, and personal vehicle for making home visits.

Qualifications
- Demonstrates leadership qualities, good interpersonal and communication skills, and teambuilding skills required.
- Keyboarding skills required for automated computer systems, including electronic medical records.
- Demonstrates knowledge of the principles of life span growth and development and end of life.
- Interprets the appropriate information needed to identify each patient’s requirements relative to his or her age specific needs and provides care according to departmental policies and procedures.
