RN / Registered Nurse - Home Health - Float

Adoration Home Health is seeking a passionate, dedicated Home Health RN to join our team in Louisville, KY. Our Home Health RNs provide expert, patient-centered care in a supportive, fulfilling environment.

About the role

As a Home Health Registered Nurse, you will deliver 1:1 care to patients in the Louisville market, offering greater work/life balance with flexible scheduling, less time on your feet compared to other settings, and the ability to work independently while receiving team support. This is a full-time float position based in our Louisville office.

Responsibilities

• Assess and monitor the physical, emotional, and psychological needs of patients.
• Create home health care plans that align with MD orders and the patient's goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.

Requirements

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and residence.
• One year of nursing practice in a patient care setting required; home health, geriatrics, or other related settings preferred.
• Valid driver’s license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
• Current CPR certification.

New nursing graduates may be considered in select markets based on program availability.
