RN (Registered Nurse) Home Health Admissions

As a Phoenix Admissions RN, you will serve as the initial point of contact between a patient and the hospital, performing physical assessments, interpreting diagnostics, and communicating effectively to develop an individualized care plan based on physician orders.

Responsibilities

• Skilled Visits: Day-to-day patient interaction, education on health conditions like CHF, COPD, and diabetes, wound care, medication management, changes dressings, bandages, and contraptions as prescribed, documentation of patient’s condition, reporting significant changes to the physician.
• Collaboration: Working alongside other nurses, Physical Therapist, Speech Therapist, and/or Occupational Therapist.

Requirements

• Missouri or Compact Nursing License.
• Strong organizational and self-management skills.
• Valid driver's license.

Pay

$75,000 - $90,000 per year.

Monday - Friday, Days.
