Job Duties
- Coordinates the treatment plan coordination for Hematology and Oncology patients through the continuum of care.
- Provides ongoing patient care and education for patients in the adult Hem/Onc Department.
- Serves as a resource person for patient and family regarding the perception of the diagnosis and medical plan of care for the management of disease as designated by the physician.
- Serves as a patient liaison between the patient and the care team as they encounter issues/obstacles/changes to their plan of care.
- Performs pre-visit RN triaging for new and existing patients to ensure that decisions can be made on the first visit and treatment planning for the Hematologic/Oncologic care can be initiated.
- Affords supportive care referrals such as referrals to Palliative care, Psychology, and social work.
- Directly responsible for the completion and documentation of the initial chemotherapy teach session in EPIC as required by regulatory bodies.
- Participates in performance improvement activities to ensure high quality care delivery and assists providers with completion.
- Affords assistance with orientation of new personnel to ensure that they are appropriately trained to support patients and providers.
- Executes and documents ongoing patient communication.
- Provides instruction to ensure patient's understanding of medical condition and compliance with treatment instructions.
- Performs and assists with triaging telephone calls concerning health issues and offers appropriate telephone management advice to patients as liaison to physician recommendations.
- Communicates lab, radiology and other testing results to patients and via telephone and electronic medical record letter notification.
