Responsibilities
- Assess patients' medical needs
- Develop individualized care plans
- Ensure appropriate symptom management, medications, and therapies
- Maintain comfort and dignity for patients
- Serve as the primary clinical liaison between patients, families, and the interdisciplinary hospice team
- Provide education and emotional support to families
- Navigate complex medical decisions
- Avoid non-compliance with regulatory standards
- Advocate for the patient's wishes
- Enhance quality of life during hospice care
Requirements
- Bachelor's degree in Nursing (RN)
- Current licensure as a Registered Nurse in the state of Kansas
- Experience in hospice or palliative care preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of hospice policies and procedures
Qualifications
- Excellent organizational and time management skills
- Proficiency in Microsoft Office Suite
- Ability to handle sensitive and confidential information
