About the role

Grane Hospice is seeking a passionate, dedicated Hospice RN to join their team in York, PA. The RNs provide expert, patient-centered care at a critical time in people’s lives.

Responsibilities

  • Provide 1:1 care to make a lasting impact on patients and families
  • Offer greater work/life balance with flexible scheduling options

  • Assess and monitor physical, emotional, and psychological needs of patients
  • Create hospice care plans aligned with patients' wishes and goals
  • Direct nursing care including administering medications, treatments, and interventions
  • Pain and symptom management
  • Support the patient's family and caregivers
  • Collaborate with interdisciplinary team
  • Maintain accurate and timely documentation
  • Participate in on-call rotation as required by local branch

Qualifications

  • Graduate of an accredited nursing school with current licensure to practice in state of operation
  • One year nursing experience required, three years preferred
  • Experience in hospice or a similar setting preferred
  • Knowledge of the hospice philosophy of care
  • Commitment to clinical and documentation excellence
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
