RN / Registered Nurse Case Manager - Hospice

Our Company Generations Hospice is seeking a passionate, dedicated Hospice RN to join our team! Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. If you’re ready to work in a supportive, fulfilling environment where your skills and empathy truly shine, we offer a $5,000 Sign-On Bonus. Office Location: 1028 S. Main Street Suite C Fond du Lac, WI 54935. Coverage area: Travel throughout the territory is required - Serving Winnebago & Fond du Lac Counties. Schedule: Monday-Friday 8AM-5PM.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
