RN / Registered Nurse Case Manager - Hospice
Office Location: 8751 Park Central Drive Suite 140, Richmond, VA 23227. Travel required throughout the Richmond, VA location territories.
About the role
AT Home Care Hospice is seeking a passionate, dedicated Hospice RN to join our team. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. A $5,000 sign-on bonus is available to external/new hires only; repayment terms will be outlined in the offer letter.

Responsibilities
- Assess and monitor physical, emotional, and psychological needs of patients
- Create hospice care plans that align with the patients’ wishes and goals
- Direct nursing care: administering medications, treatments, and interventions
- Provide pain and symptom management
- Educate and support the patient’s family and caregivers
- Collaborate with interdisciplinary team
- Maintain accurate and timely documentation
- Participate in on-call rotation as required by local branch
Requirements
- Graduate of an accredited nursing school with current licensure to practice in state of operation
- One year nursing experience required; 3+ years preferred
- Experience in hospice or a similar setting preferred
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
- Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order
Pay
USD $39.00 - $40.00 / hour
Schedule
Full Time, Monday–Friday, 8 AM–5 PM
