RN / Registered Nurse Case Manager - Hospice - $5,000 SIGN ON BONUS

Office Location: 735 Cornerstone Crossing, Waterford, WI 53185. Coverage area: Travel throughout the territory is required, including Waterford, Racine, Kenosha, Walworth, and southeastern Milwaukee County. Schedule: Full Time Monday-Friday 8AM-5PM.

Responsibilities

As a RN Case Manager, you will:

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Qualifications

To be considered, you must be a:

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• Have one year nursing experience required, 3+ years preferred
• Have experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
