RN Registered Nurse Case Manager Responsibilities

• Assess patients' physical and emotional needs in their homes and develop individualized care plans to ensure optimal health outcomes
• Coordinate and collaborate with patients, families, caregivers, and healthcare providers to ensure seamless continuity of care
• Provide skilled nursing care, including wound care, medication management, and education to patients and caregivers
• Conduct regular home visits to monitor patients' progress, identify potential breakthroughs, and make adjustments to care plans as needed
• Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate transmission of patient information
• Develop and maintain accurate and up-to-date patient records and reports
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction
• Collaborate with interdisciplinary teams to develop and implement patient-centered care plans

RN Registered Nurse Case Manager Requirements

• Current licensure as a Registered Nurse in the State of Pennsylvania
• Prior experience in home health care, case management, or a related field
• Strong clinical and case management skills, with the ability to assess patient needs and develop effective care plans
• Excellent communication, interpersonal, and organizational skills
• Able to work independently and as part of a team
• Possess a valid driver's license and reliable transportation
