About the role
Registered Nurse providing in-home care for Pediatric and/or Adult patients.
Responsibilities
- Utilize the nursing process to plan, implement, and evaluate patient care and services.
- Develop and revise care plans based on patient needs and physician directives.
- Apply therapeutic, preventive, and rehabilitative nursing actions aligned with patient diagnoses and goals.
- Respond to changes in patient condition, reporting to physicians or supervisors as needed.
- Administer medications and treatments, monitoring their effectiveness.
- Review care plans, identify educational opportunities, and assist with care coordination and discharge planning.
Qualifications & Skills
- Current RN license for the practicing state.
- Valid health certificate, PPD or chest X-ray, and BLS certification.
- Preferred: at least one year of RN experience.
Key Duties
- Deliver patient-centered nursing care in a home setting.
- Collaborate with healthcare teams to ensure optimal patient outcomes.
- Maintain accurate documentation of treatments and patient responses.
