Brief Description
Under the direction of the Scranton Transitional Director of Nursing, the Patient Onboarding Nurse coordinates the new admission’s transition into the Scranton Transitional Unit. This position facilitates a positive first impression to the new patient, and their representative, if applicable, through facility and staff introductions, orientation, and utilization of strong customer service skills during this transition period.
Responsibilities
- Evaluates and documents Braden Scale, bowel and bladder, pain assessment, medication review, and coordination with pharmacy/physician upon admission.
- Coordinates with attending physician for order entry into the Point Click Care EMR system.
- Provides introductions to the professional nursing staff and direct care staff assigned to the resident within 2 hours of admission.
- Describes facility normal routines and obtains the resident’s customary routine and preferences.
- Facilitates creating a homelike environment and securing personal possessions upon entry.
- Develops the baseline care plan within 48 hours of admission in collaboration with the resident, and representative as appropriate, and other disciplines.
- Ensures the care plan interventions are in place to create a smooth transition to this level of care, incorporating the goals of the resident/representative.
- Provides exceptional customer service skills to residents, visitors, and other team members.
- Participates in the unit’s QAPI program and provides feedback to administration on potential performance improvement areas; facilitates resident and family feedback on the same.
- Works collaboratively with administration to identify solutions and participates with monitoring effectiveness.
- Provides input and feedback into the quarterly facility assessment.
Requirements
- Graduate of an accredited Nursing Program required.
- Minimum of one (1) year of experience preferred as an RN.
- Possession of a current license as a Registered Nurse as issued by the Pennsylvania State Board of Nurse Examiners.
- Ability to interact and communicate effectively both verbally and in writing required.
- Proficient in data entry into the EMR system.
- Strong time management skills.
- CPR certification must be obtained and maintained.
- Must be cleared by PA State Police criminal history check.
- Must be able to demonstrate the knowledge and skills necessary to provide individualized care appropriate to the age specific needs and culture of the patient served.
- Participation in formal staff development programs.
