RN Patient Navigator BMT/Cell Therapy

About the role

You’re the life preserver for countless patients because, as a Patient Navigator Nurse, you’re the hub of your patients’ multidisciplinary team advocating, interpreting, coordinating, educating, supporting, and counseling along their care journey. As a clinical resource with expertise in specialty care management, you are that critical connection and liaison for a patient population recovering from strokes, cancer, and long-term illnesses. You remain positive, upbeat and dedicated to assuring quality and focusing on achieving the best possible outcomes for your patients. Responsibilities

• Coordinate and communicate written and verbal information between the patient, family, physician, physician staff, and other team members.
• Coordinate and establish the continuity of patient care with internal and external departments and agencies.
• Aid patients in understanding their diagnosis, treatment options, and available resources.
• Review medical documentation to assist physicians in obtaining complete and accurate documentation of their medical treatment plan.
• Act as a key link between patients and all other care providers, following patients throughout the care continuum.
• Collaborate with care management resources and facilitate appointments for consults and support services including social workers, PT, OT, ST, dietitians, physicians, financial resources, etc.
• Collect and track data, support strategic planning processes, and evaluate patient care plans.
• Provide guidance and support to patients and their families in making difficult decisions related to treatment.
• Update policies related to service provided to enhance professional practice.
• Participate in community educational events and staff education sessions.

Requirements

• Current nursing license to practice in Louisiana and as defined by the Louisiana State Board of Nursing.
• American Heart Association BLS certification.
• Minimum of 3 years of professional nursing experience in specialty areas.
