RN Patient Care Manager - Office-Based

About the role

This position supervises and coordinates clinical services while ensuring high-quality home health care. The Patient Care Manager RN provides and directs nursing care within the scope of practice, in accordance with physician plans and regulations.

Responsibilities

• Completing assessments, medication reconciliation, and evaluations
• Receiving referrals, assigning clinicians, evaluating patient orders, and initiating care
• Managing patient benefits, medical necessity, and insurance approvals
• Assessing patient needs continuously and tailoring care plans accordingly
• Reviewing assessments and care plans daily, advising clinicians as needed
• Operating effectively in diverse home environments regardless of patient background

Qualifications

• Unrestricted RN license in the practice state
• Valid driver's license, vehicle insurance, and access to reliable transportation

Preferred skills

• CPR certification or ability to obtain within 90 days
• Home care experience, independence, strong communication, organization, and writing skills
