RN Patient Care Manager - Hybrid Ormand Beach FL

Primary Responsibilities

• Directly/indirectly supervises home health aides and LPNs, provides instruction, and assigns tasks
• Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team
• Completes comprehensive assessments (OASIS), medication reconciliation, and initial/comprehensive nursing evaluation visits
• Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and plots start of care visits
• Congntrates determination of patient home health benefits, medical necessity, and ongoing insurance approvals
• Sures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders
• Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate

Required Qualifications

• Current unrestricted RN licensure in state of practice
• Current CPR certification requirements
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Preferred Qualifications

• Home care experience
• Able to work independently
• Good communication, writing, and organizational skills

Pay and Benefits

• Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.

• The salary for this role will range from $71,640 to $107,460 annually based on full-time employment.
