Responsibilities

  • Work collaboratively with physicians, staff, and other health care professionals to provide a medical home and care coordination across the health care continuum for all patients within the physician office setting.
  • Serve as an integral member of the health care team, ensuring safety, best practices, and high-quality standards of care are maintained across the continuum.
  • Coordinate a wide range of self-management support and disease registry activities for the clinic's entire patient population.
  • Assist in routing patients to complementary support services, as well as education and outreach programs.

Requirements

  • Graduate from a nursing program.
  • 3 years of clinical experience in chronic disease states, with demonstrated clinical competence.
  • Current licensure to practice Nursing in Georgia upon hire.
  • RN - Registered Nurse - Georgia State Licensure and/or NLC/eNLC Multistate Licensure upon hire.
  • CCTM: Care Coordination Transition Management certification upon hire.
