About the Role

Provides direct and indirect patient care in the office setting to patients, families, and caregivers in the Medical Group practice. Uses the nursing process consistent with acceptable standards of the profession and MVHS Medical Group. Provides high-quality patient-centered care that reflects initiative, flexibility, and responsibility with minimal administrative supervision. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. Communicates with outpatient clinic physicians and NP/PAs about changes in patient’s status, symptomatology, and results of diagnostic studies. Responds quickly and accurately to changes in condition or response to treatment.

Responsibilities

  • Support the Primary Care Provider (PCP) in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need.
  • Participate in the daily huddle to identify individual patient needs for upcoming appointments.
  • Serve as a clinical liaison between patient and provider for questions, concerns, and testing results.
  • Communicate effectively with other members of the care team in coordinating the individualized plan of care.
  • Support PCP during in-office procedures and examinations.
  • Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management).
  • Perform duties within scope of practice to the highest level of licensure.
  • Provide direct support and guidance to other members of the care team as appropriate.
  • Perform assessment data collection on all patients utilizing tools/forms in the Electronic Medical Record (EMR).
  • Adequately assess and reassess pain; educate the patient and family regarding pain management.
  • Provide age-appropriate care to patients in the community setting.
  • Consistently document procedures completed in the EMR, including medications administered and follow-up assessments.
  • Deliver medication utilizing the five rights and observe patient for desired and untoward effects.
  • Maintain current knowledge of medications and their correct administration.
  • Establish priorities of patient care based on needs and office resources.
  • Demonstrate the ability to assist providers with procedures and perform services requiring technical and manual skills.
  • Perform other duties as required.

Requirements

  • Graduate of an accredited school of nursing or nursing program.
  • Current New York State Registered Nursing registration.
  • BLS Certification.
  • Minimum one year of nursing experience.
  • Basic computer and EMR knowledge.
  • Excellent communication and interpersonal skills.
  • Ability to assimilate information from many different sources.
  • Must be able to communicate in English both verbally and in writing.

Qualifications

  • BSN from accredited nursing program.
  • Experience in a specialty medical office.
  • Certifications applicable to the assigned office’s specialty.

Pay

$29.78 - $50.05 per hour.

Schedule

  • Full-time, non-union, 8-hour day shifts.
  • FTE: 1.0
