About the role
The RN-On Call Nurse is responsible for providing hospice nursing care, coordinating with healthcare providers, maintaining accurate documentation, and ensuring the patient and family receive appropriate support.
Responsibilities
- Absorbs responsibility for a patient visit or on-call schedule including assessing, planning, implementing, and evaluating the nursing process.
- Provides hospice nursing care focusing on pain control and symptom management.
- Communicates with attending physicians, other hospice staff members, and other agencies to coordinate care and utilize resources for the patient/family.
- Maintains regular communication with the direct supervisor to review assignments.
- Maintains up-to-date documentation at the point of service to accurately reflect problems, plans, actions, and goals, and charges.
- Administers medication according to federal and state laws and Hospice policies, along with LTC staff.
- Maintains current patient records and documents in accordance with Hospice policies to ensure accurate and clear statements of problems, interventions, goals, and outcomes, and charges.
- Assesses and collaborates with the Medical Director to determine initial and ongoing eligibility of the hospice patient.
- Completes an initial assessment of the patient and family to determine home care needs.
- Anticipates the end-stage disease process and related pain and symptom management.
- Regularly re-evaluates the patient's nursing needs and uses assessment data to determine care plan needs.
- Initiates appropriate preventive and palliative nursing procedures.
- Counsels the patient and family in meeting end-of-life needs.
- Completes a comprehensive death note when appropriate.
- Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition.
- Communicates with other members of the interdisciplinary team to coordinate the care plan.
- Participates in on-call duties as defined by the on-call policy.
- Ensures that arrangements for equipment and other necessary items and services are made.
Requirements
- Associate's Degree in Nursing.
- State Certified RN license.
- Experience in hospice or home health care, REQUIRED.
- Excellent documentation skills and the ability to work independently.
- Strong communication skills.
- Valid driver's license and reliable transportation.
- Available to travel locally within assigned territory.
