RN On-call
Hearth Hospice is seeking a Per Diem On-Call Registered Nurse (RN) to provide exceptional care and support to hospice patients and their families during critical moments. This role focuses on identifying patient and family needs, coordinating care, and offering compassionate support.
About the Role
This per diem position provides coverage for weekends and PTO. The territory for this role is Cleveland.

Responsibilities
- Respond to a variety of patient needs, from urgent to routine, providing critical support in key moments.
- Conduct comprehensive assessments of patients' physical, emotional, social, and spiritual needs to foster meaningful connections.
- Maintain accurate and detailed documentation to ensure high-quality care in compliance with agency and regulatory standards.
- Support families by sharing expertise, guiding them in caring for their terminally ill loved ones, and offering reassurance during challenging times.
Requirements
- Bachelor's degree in nursing or healthcare preferred.
- Current unencumbered license as a Registered Nurse (RN).
- Minimum one (1) year of experience as an RN in a medical-surgical or acute care setting.
- Hospice experience is a plus.
- Computer proficiency, including experience with typing and Electronic Medical Records (EMR).
- Current CPR certification, if required by state.
Pay
$32.00 - $38.00 per hour. The pay range is a general guideline and may vary based on the candidate’s relevant experience and the specific responsibilities of the position. Final compensation will be discussed and confirmed during the interview process.
