RN Nurse Coordinator - Neurosurgery

About the role

Provides advanced nursing support as part of an interdisciplinary team to promote preventative care, conduct screenings, and manage disease programs. Offers primary clinical assistance to providers, patients, and families for diagnosis and treatment, including triaging patient communications and creating individualized care plans.

Responsibilities

• Assess healthcare, educational, and psychosocial needs, involving patients and families in treatment planning.
• Facilitate communication among healthcare team members and explore healthcare options to eliminate barriers.
• Educate patients and families on disease management, care coordination, and program outcomes using age-appropriate techniques.
• Implement clinical interventions based on evidence-based guidelines and risk stratification.
• Collaborate with primary care providers to ensure guideline adherence, initiate therapies, and provide follow-up.
• Coordinate diagnostic and laboratory testing, document activities in electronic health records, and collect patient data.
• Participate in ongoing professional development and incorporate current research into practice.
• Support post-discharge care transitions, assist in designing care programs, and prepare lab specimens.
• Administer medications, vaccines, and perform IV management within scope of practice.

Qualifications

• Graduate from a Specialty Training Program in Nursing (required)
• Bachelor's Degree in Nursing (preferred)
• Minimum of 2 years RN experience required
