RN Navigator - Cardiac Services

About the Role

As an RN Navigator for Cardiac Services, you will serve as a key liaison for cardiac patients and their families, ensuring coordinated care and guiding them through their treatment journey. You will collaborate closely with cardiologists, surgeons, and multidisciplinary teams to streamline care pathways, monitor patient progress, and proactively address needs to support safety and adherence to evidence-based practices. This role offers an opportunity to support advanced cardiac care in a high-acuity setting and enhance service cohesion across the health system.

Responsibilities

• Act as a primary clinical resource for cardiac patients and their families.
• Facilitate communication across departments to ensure coordinated care.
• Monitor patient progress and proactively address needs to support safety and adherence to evidence-based practices.
• Contribute to performance improvement and patient experience initiatives within the cardiac program.

Qualifications

• High School Diploma or equivalent; completion of an IEP Certificate preferred.
• Minimum of 3 years recent nursing experience in Cardiology, Pulmonology, or Cath Lab/Care Coordination.
• Valid RN license for California.
• Current BLS certification from AHA.
• Bachelor’s degree in Nursing (BSN) or relevant national certification required.
