Responsible for guiding patients and families through complex care journeys from diagnosis to survivorship or chronic disease management.

Responsibilities

• Coordinate appointments, tests, and referrals to ensure seamless care delivery
• Provide evidence-based education to patients and families
• Collaborate with physicians and multidisciplinary teams to address and remove barriers to care
• Support quality outcomes, enhance patient satisfaction, and facilitate smooth care transitions

Qualifications And Skills

• Registered Nurse (RN) licensure
• Strong communication and collaboration skills
• Ability to educate, coordinate, and advocate for patients
• Commitment to compassionate, team-oriented healthcare
