About the role
RYZE at Homewood is seeking a dedicated Licensed Practical Nurse (LPN) / Registered Nurse (RN) to join our compassionate care team at our skilled nursing facility in Homewood, Illinois. Duties and Responsibilities
- Provide comprehensive nursing care to residents during the PM shift.
- Assess resident conditions and collaborate with the interdisciplinary team to develop, implement, and update individualized care plans.
- Administer medications, treatments, and therapies as ordered by the physician.
- Monitor residents for changes in condition and promptly communicate concerns to the appropriate healthcare provider.
- Supervise Certified Nursing Assistants (CNAs) and other assigned staff, providing guidance and support throughout the shift.
- Delegate nursing assignments appropriately and ensure quality resident care is delivered.
- Accurately document resident assessments, treatments, medication administration, and significant observations in the electronic medical record.
- Educate residents and family members regarding medications, treatments, and care plans.
- Serve as Charge Nurse by addressing clinical concerns, supporting staff, and coordinating resident care during the shift.
- Ensure compliance with infection prevention protocols, safety procedures, and all federal, state, and facility regulations.
- Collaborate with physicians, therapists, social services, dietary, and other departments to provide high-quality resident care.
- Participate in shift reports, care conferences, and required in-service education.
- Perform other duties as assigned.
Skills and Qualifications
- Graduate of an accredited nursing program.
- Current Illinois Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing.
- Previous skilled nursing, rehabilitation, or long-term care experience preferred.
- Current CPR certification preferred or required in accordance with facility policy.
- Strong assessment, clinical judgment, and critical thinking skills.
- Excellent communication, leadership, and organizational abilities.
- Ability to supervise nursing staff while promoting teamwork and quality care.
- Ability to work effectively with geriatric and rehabilitation populations.
- Ability to perform the essential functions of the position with or without reasonable accommodation.
- Must successfully complete all required background checks and pre-employment screenings.
