About the role
Horizon Health is seeking a full-time Registered Nurse, Licensed Practical Nurse or Certified Medical Assistant to provide nursing care in an outpatient clinic setting. Candidates should be passionate about delivering high-quality, patient-centered care.
Responsibilities
- Care coordination for patients throughout their visit, including rooming, taking vitals, gathering medical history, updating EHR, preparing for provider visits, scheduling follow-ups, and cleaning/stocking rooms.
- Patient education, including health management, referral management, care coordination for preventive and specialty visits, and development of care plans, including self-management plans and goal initiation.
- Performing nursing skills as per scope of license, including injections, vaccinations, labs, x-rays, and other tests. On-the-job training provided for labs and x-rays.
Requirements
- Valid SD Nursing License for RNs
- Certified through an accredited program for Medical Assistants
