Horizon Health in Woonsocket, SD is seeking a full-time Registered Nurse, Licensed Practical Nurse or Certified Medical Assistant to provide nursing care to patients in an outpatient clinic setting. The role involves duties split between front office and nursing care, ideal for candidates passionate about delivering high-quality, patient-centered care in a collaborative, team-oriented environment.
Responsibilities
- Coordinate care for patients through the entire visit, including rooming patients, taking vitals, collecting medical history, updating medications, and documenting clinical information in the EHR.
- Prepare patients for provider visits, schedule follow-up care, and ensure rooms are cleaned and stocked.
- Provide patient education on health management, referral coordination, preventative/specialty care, and care plan development, including self-management goals.
- Perform nursing skills within scope of license, such as administering injections, vaccinations, labs, x-rays, and other tests (on-the-job training provided).
- Manage appointment schedules to ensure appropriate time allocation and provider assignments per organizational guidelines.
- Collect and verify patient demographic and insurance information confidentially.
Requirements
- Valid South Dakota Nursing License (for RN/LPN) or certification from an accredited program (for Medical Assistants).
