MDS Coordinator Summary
You'll serve as the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) Coordinator for our center. Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines.
MDS Coordinator Responsibilities
- Collaborating with nursing caregivers and other disciplines to ensure accurate care plan goals
- Educating nursing caregivers on the importance of accurate MDS assessments
- Ensuring timely completion and transmission of RAI/MDS forms
- Preparing accurate reports on Quality Measure/Quality Indicators (QM/QI) from RAI/MDS data
- Communicating with various disciplines to facilitate timely completion and submission of MDS assessments

Qualifications
- Bachelor's degree in Nursing (RN) or Licensed Practical Nurse (LPN)
- Current licensure as an RN or LPN in the state of Michigan
- Experience in long-term care or skilled nursing facility preferred
- Strong organizational and communication skills
- Ability to work independently and as part of a team
Skills
- Excellent interpersonal and communication skills
- Proficiency in Microsoft Office Suite
- Knowledge of Medicare and Medicaid regulations
- Ability to manage multiple tasks and meet deadlines
