RN/LPN MDS Coordinator- Experience Required

MDS Coordinator Summary

You'll serve as the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) Coordinator for our center. Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines.

MDS Coordinator Responsibilities

• Collaborating with nursing caregivers and other disciplines to ensure accurate care plan goals
• Educating nursing caregivers on the importance of accurate MDS assessments
• Ensuring timely completion and transmission of RAI/MDS forms
• Preparing accurate reports on Quality Measure/Quality Indicators (QM/QI) from RAI/MDS data
• Communicating with various disciplines to facilitate timely completion and submission of MDS assessments

Qualifications

• Bachelor's degree in Nursing (RN) or Licensed Practical Nurse (LPN)
• Current licensure as an RN or LPN in the state of Michigan
• Experience in long-term care or skilled nursing facility preferred
• Strong organizational and communication skills
• Ability to work independently and as part of a team

Skills

• Excellent interpersonal and communication skills

• Knowledge of Medicare and Medicaid regulations
• Ability to manage multiple tasks and meet deadlines
