RN II PHYSICAL REHAB CARE

About the Role

As our Rehabilitation Nurse, you will help high-acuity patients restore their health so they can care for themselves at home and resume their lives with family. Every day you will utilize a wide range of nursing skills and provide comprehensive patient and family education. You will advocate for patient needs, deliver tailored care, and embrace a dynamic, non-routine environment.

Responsibilities

• Assess patients and initiate Nursing Assessment within the number of hours standard on the unit, ensuring physician orders are implemented promptly and accurately.
• Formulate care plans using nursing diagnosis, planned outcomes, and nursing judgment related to patient/family needs; initiate discharge planning.
• Identify deviations from expected vital signs and lab tests; document, report, and follow through on abnormal findings with physicians/nurses.
• Document nursing assessments, interventions, patient responses, and outcomes accurately and concisely; provide clear reports on patients.
• Administer medications, including narcotics, following hospital policies; demonstrate knowledge of dosage, route, action, side effects, and contraindications.
• Identify drug interactions, know antidotes/treatment for medication overdose and infiltration; calculate dosages accurately and document medication administration appropriately.
• Set priorities, organize, and complete care for assigned patients within shift; reorganize and reprioritize workload to meet changes in unit activity.
• Make independent decisions; seek assistance in critical, complex situations; give accurate and organized reports.
• Maintain cost consciousness by efficiently using supplies and adhering to patient chargeable billing systems.
• Adapt behaviors based on peer and Nurse Manager/Supervisor evaluations; maintain patient confidentiality.
• Follow through on less predictable problems, refer to appropriate disciplines, update care plans, and review target dates.
• Involve patient/family in care, establish teaching plans, coordinate discharge plans, attend physician rounds, and participate in patient care conferences.
• Anticipate educational needs of patients and families; plan and provide instruction adapted to their level of understanding, including written resource material.
• Document instruction and patient/family responses; update nursing care plans based on analysis of patient responses.
• Collaborate with multidisciplinary teams in planning patient education and discharge planning.

Requirements

• Six months of current medical/surgical nursing experience in an acute care setting or specialty area.
• Active California RN License.
• Basic Life Support (BLS) certification within 30 days of hire.
• Advanced Cardiac Life Support (ACLS) certification within 30 days of hire.

Preferred Qualifications

• Certified Rehabilitation RN (CRRN).

Pay

$51.57 - $76.96 per hour.
