About the role

The RN Assessor plays a vital, member-facing role within Camden County, focusing on Camden City and surrounding areas like Pensauken and Cherry Hill. This position is responsible for conducting in-home assessments for individuals seeking services such as Personal Preference Programs (PPP), medical daycare (adult and pediatric), and personal assistance programs. The role involves managing a 7-day turnaround for evaluations, typically completing 14-16 member assessments per week. Assessments are conducted primarily in members' homes, evaluating ADLs and IADLs, and require both observation and interview-based data collection.

Responsibilities

  • Assesses patient's clinical need against established guidelines and/or standards to ensure medically appropriate services.
  • Evaluates the necessity, appropriateness, and efficiency of services provided.
  • Develops, coordinates, and assists in implementing services to meet members' needs.
  • Coordinates care with patients, families, physicians, hospitals, and other external customers.
  • Maintains continuity and efficiency in managing member care across the continuum of care.
  • Educates members on available services and supports them in utilizing DSNP if applicable.
  • Documents accurately and comprehensively based on clinical practice guidelines and organizational policies.
  • Communicates effectively with multidisciplinary teams, both telephonically and in person.
  • Evaluates care through problem-solving, analyzing variances, and participating in quality improvement initiatives.
  • Completes other assigned functions as directed by management.

Qualifications

  • Requires an associate's or bachelor's degree (or higher) in nursing and/or accredited diploma nursing school.
  • A minimum of two (2) years clinical experience is preferred.
  • Active Unrestricted NJ RN License Required.
  • Knowledge proficiency in personal computers and Microsoft Office products.
  • Working knowledge of case/care management principles, utilization management, health care contracts, and benefit eligibility requirements.

Travel

This is a field-based position requiring daily local travel. A valid New Jersey State License and insurance are required.

Salary Range

$70,500 - $94,395
