About the role
Masonicare Home Health & Hospice seeks a dedicated and experienced Registered Nurse to provide comprehensive care to clients in Wallingford, CT. The ideal candidate will have a strong background in home health care and a commitment to providing quality, compassionate care.
Responsibilities
- Accepts appropriate referrals to the agency.
- Sets up qualified professional personnel to meet the needs of clients.
- In collaboration with clinical staff, creates on-call schedules and assigns daily tasks.
- Supervises hospice and palliative nursing personnel, ensuring they deliver high-quality care.
- Assigns work to subordinate staff, provides guidance, and reviews their work.
- Coordinates and manages all professional services provided by agency staff.
- Maintains a balanced caseload and organizes work flow, utilizing clinical staff as needed.
- Performs periodic home visits to model behavior, solve problems, supervise, and evaluate performance.
- Participates in the professional development of assigned staff through orientation, skill development, and mentoring activities.
- Maintains an office environment that supports the professional growth of assigned staff.
- Assists in staff recruitment, selection, and discipline/termination of employment.
- Conducts clinical record reviews to assess the appropriateness of care plans, service utilization, and documentation.
- Engages in program planning and evaluation activities related to clinical services.
- Timely and accurately enters patient/job information into the system to support the development of the OASIS data set, hospice item sets, orders for service, time sheets, and other required documents and reports.
- Participates in administrative call on a rotating basis with other members of the management team.
- Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities, and physical condition.
Qualifications
- Education: A Baccalaureate Degree from an NLN accredited School of Nursing or a Diploma in nursing plus a minimum of three years of full-time or full-time equivalent clinical experience in a home health agency or community health program, with at least one year in a home health setting.
- Experience: Knowledge of State Regulations, Medicare and Medicaid standards as related to Hospice services.
- Certifications/Licenses: Current Connecticut Registered Nurse License.

The pay rate for this position is $X per hour, depending on experience and qualifications.
Schedule
This position works 24 hours per week and is compensated at a rate of 32 hours per week.
