About the role
The RN Hospice Nurse ensures quality and safe delivery of hospice nursing services to patients in the home or assisted living setting. Responsibilities include developing, implementing, and evaluating the plan of care, coordinating with other health professionals, and supervising Licensed Practical Nurses and Home Health Aides. The role also involves educating and counseling clients and their families, ensuring their involvement in the plan of care, and maintaining timely and accurate electronic documentation.
Responsibilities
- Develop, implement, and evaluate the plan of care, making necessary revisions to address all problems.
- Coordinate the plan of care, maintaining continuity with other health professionals and identifying interdisciplinary care needs.
- Supervise client care rendered by Licensed Practical Nurses and Home Health Aides, conducting supervisory visits per regulatory guidelines.
- Provide education, supervision, and counseling to clients and their families regarding home nursing procedures and care needs.
- Involve the client and their family in the development of the plan of care whenever possible.
- Demonstrate timely and accurate computer documentation of patient/job information for OASIS data set, orders, time sheets, and other required documents.
Requirements
- Current RN License required.
- Hospice and/or Home Care experience is preferred.
- New graduates will not be considered.
