Hospice Corewell Health specializes in providing end of life care and symptom management to individuals with a life expectancy of six months or less. Summary
The RN is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet the patients/family's end-of-life care needs. A comprehensive plan of care addressing physical, emotional, cultural, and spiritual needs is developed and continuously adjusted through the nursing process, relationship-centered care principles, and a holistic philosophy, ensuring the changing needs of the patient/family are met while facilitating a pain-free, peaceful, and sacred death.
Essential Functions
- Exhibit “Patients and Families First” attitude. Respond with compassion, flexibility, and urgency to each patient’s needs. Provide the care that patients want when they want it.
- Develop a comprehensive plan of care that addresses physical, emotional, cultural, and spiritual needs of the patient/family with input from the patient/family and the interdisciplinary team.
- Evaluate this plan and make interventions as necessary.
- Advocate for pain/symptom management.
- Provide education and support to the patient/family regarding care issues.
- Committed to the continuous improvement of the patient experience which includes patient safety, quality of care, and excellent service.
- Adhere to the organization’s Mission, Service Standards, and Code of Conduct.
- Direct and supervise LPN and CENA.
- Administer and document medications/treatments as ordered by the physician according to policies in an accurate and timely manner.
- Identify and respond appropriately to emergency situations.
- Attend shift reporting and IDT meetings as necessary to facilitate communication of decisions related to the patients' care.
- Participate actively in safety initiatives and risk mitigating measures where appropriate and complete all position and unit safety-related competencies and requirements on a timely basis.
- Perform other duties as assigned.
Qualifications
- Associate’s degree in nursing
- One year of relevant experience, preferably in Hospice
- Michigan Registered Nurse (RN) license upon hire
- CPR (AHA)
