RN - Hospice Nurse (Camden/Gloucester County) + $5,000 Sign-On Bonus

About the role

As a Hospice Nurse, you will be responsible for overseeing the quality and expert care of your assigned hospice patients.

Responsibilities

• Perform patient assessments for comfort and relief of symptoms.
• Establish and implement an individualized plan of care for each patient.
• Assess the need for patient and family education and provide education as needed.
• Obtain appropriate physician orders to achieve symptom management.
• Triage and arrange home visit schedule, demonstrating efficiency in time and mile management.
• Make referrals to additional IDT members and collaborate with social workers related to their involvement.

Requirements

• Minimum of two years of nursing experience.
• NJ RN license required; BSN preferred
• Bilingual is a plus!

Pay

$80,808 – $93,712.51 annually based on years of experience + $5,000 Sign-On Bonus
