RN Hospice Case Manager Float
About the role
The Hospice RN Case Manager coordinates care and provides initial and ongoing nursing assessment of the patient/family needs and coordination of the patient's plan of care with interdisciplinary team members and attending physician.
Primary Responsibilities
- Provides initial and ongoing assessments of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living, i.e., risk of pathological grief; cultural and spiritual implications; verbal and non-verbal communication patterns
- Assesses the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care
- Applies specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility
- Implements the individualized plan of care and recommends revisions to the plan as necessary, i.e., managing discomfort and providing symptom relief; supplying specialized nursing skills related to palliative and end-of-life care
- Consults and educates the patient/family and other caregivers regarding: the disease process; pain and symptom management; self-care techniques; end of life care; and processes for dealing with issues of ethical concern
- Initiates appropriate preventive and rehabilitative nursing procedures
- Ensures continual assessment of assigned patient's and family's needs from admission throughout the course of care
- Provides ongoing evaluation of the patient/family response to care
- Provides specialized hospice training to other staff, family members and informal caregivers to ensure safe care
- Informs the physician of patient's needs and outcomes of interventions as per standards
- Determines the scope and frequency of services needed based on acuity and patient/family needs
- Captures all patient/family services and prioritization of needs with the members of the interdisciplinary team
- Supervises LPNs/LVNs, Hospice Aides, and other paraprofessionals providing services to the patient according to regulatory guidelines
- Documents progress toward established goals as per standards
- Uses the case management approach and refers to other services as needed
- Maintains a patient case load, daily visits and point of care documentation levels as per standards
- Communicates information using current process and technology available to the organization
- Reports necessary information to Clinical Services Manager or designee
- Participates with the on-call rotation schedule and weekends as assigned
- Completes all job duties in a manner that meets the accepted standards of practice and the Optum Palliative and Hospice Care Policies and Procedures
- Provides nursing services in accordance with accepted standards of practice and within the scope of the state specific Nurse Practice Act
- Works collaboratively with others to achieve goals
Required Qualifications
- Current certification to practice as a Licensed Registered Nurse in the state of the hospice
- Demonstrated experience of excellent nursing skills
- Knowledge of clinical manifestations of diseases and ability to develop hospice specific clinical knowledge
- Reliable transportation
- Proven excellent verbal and written communication skills
Preferred Qualifications
- BSN
- Hospice & Palliative Care Nursing certification
- 1+ years of hospice experience within the last three years or 1+ years of clinical experience in hospital, home health or community setting
- Proficient computer skills and Electronic Medical Record experience
