About the role
$2,500 sign-on bonus for external candidates. As a Registered Nurse Case Manager with Elite Hospice, you will assume full nursing responsibility for delivering each patient’s Plan of Care, continuously evaluating and adjusting performance to increase productivity and quality of care. You will develop and implement treatment strategies grounded in scientific nursing theory to promote physical, psychosocial, emotional, and spiritual well-being for patients and families facing end-of-life challenges.
Responsibilities
- Conduct the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral
- Identify the patient’s and family’s physical, psychosocial, and environmental needs and reassess at least every 15 days
- Initiate and coordinate the plan of care; document problems, goals, interventions, and patient/family responses
- Collaborate daily with the patient, family, attending physician, and interdisciplinary group (IDG) to deliver care
- Instruct and supervise patients and families in self-care techniques as appropriate
- Maintain accurate and relevant clinical notes regarding the patient’s condition
- Function effectively in any home environment regardless of age, race, creed, color, sex, disability, or financial status
Required Qualifications
- Current and unrestricted RN licensure in the state of practice
- Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
Preferred Qualifications
- 1+ years of clinical experience
- Current CPR certification or ability to complete within 90 days of hire
Pay
The salary for this role ranges from $67,660 to $101,490 annually based on full-time employment, and is determined by factors including local labor markets, education, work experience, and certifications.
