Job Summary
Responsible for providing direct skilled nursing care and coordination/management of patients admitted to home care/clinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation.
What you will do:
- Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services.
- Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.
- Perform physical assessments, develop, coordinate, modify and manage written individualized plan of care, as ordered by the primary physician, to meet patient’s long and short-term medical goals and appropriate/timely home health outcomes.
- Develop and clinically manage the Home Health Aide’s care plan according to the patient’s evolving personal are needs.
- Collaborate and communicate with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.
- Perform discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
- Provide accurate, complete and timely clinical documentation in compliance with regulatory, quality and Agency guidelines.
- Aid with the plan and implementation of family Bereavement services for the following patient death.
What you bring:
- Education: Accredited School of Nursing Required: Nursing Bachelor's Degree Preferred: Bachelor of Science in Nursing
- Experience: 2 years Required: Acute clinical or home health/hospice nursing experience 1 year Required: Home health experience preferred or will orient to point of competency Required: Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient’s needs
- Credentials: Required: Registered Nurse in state of Orgon Required: Drivers License and proof of auto insurance Required: Upon Hire Basic Life Support
Working Conditions:
- Lifting Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
- Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
- Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts).
- Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA)
Environmental Conditions:
- Exposure to biohazard, body fluids and airborne particles.
- Must be able to complete tasks in a noisy environment.
Mental/Visual:
- Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
- Ability to communicate and exchange accurate information.
