About the Role
The Homecare Admission Nurse ensures quality and safe delivery of nursing and home health services. Coordinates provision of home health care services that reflect the agency philosophy and standards of home health and nursing care of assigned clients. The primary function is to admit patients to service, including medication reconciliation and completion of the OASIS data set. An Admission Nurse typically completes 2-3 admissions per day and may also perform revisits as needed to meet agency productivity.
Responsibilities
- Carry out the plan of care for the client as developed by the nurse case manager, addressing all problems in the plan, identifying interdisciplinary care needs, and making recommendations for additional home health care services.
- Assist the nurse case manager by coordinating the plan of care, maintaining continuity of client care with other health professionals, communicating changes in status, and participating in client care conferences.
- Supervise patient care rendered by Licensed Practical Nurses.
- Supervise, educate, and conduct supervisory visits for home health aides.
- Provide education, supervision, and counseling to the client and family regarding home nursing procedures and other care needs.
- Utilize agency educational material as appropriate.
- Document timely and accurate computer entry of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets, and other required documents.
- Participate in the agency’s continuous quality improvement program.
- Communicate effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities, and physical condition.
- Report to work on time, use time off appropriately, and complete annual education and medical requirements.
- Perform other duties as required.
Requirements
- RN Licensure
- Prior homecare experience
- OASIS Certification is a plus
- Clean driver’s license and reliable transportation

Schedule
Day Shift / Work 24hrs per week / Paid for 32hrs per week. Flexible weekend scheduling options: Friday, Saturday & Sunday or Saturday, Sunday & Monday.
Pay
The pay range for this position is $40.00 - $52.00 per hour, based on experience.
