RN Home Palliative Case Manager - .9 FT - Full TIme Days

About the role

The RN Home Palliative Case Manager coordinates and oversees the interdisciplinary plan of care for patients in a home care/home hospice environment. This position requires a nurse who is comfortable working independently, enjoys teaching, is a critical thinker, and has strong communication skills. The role involves providing hands-on care in the home environment and offering autonomy, flexibility, and the luxury of caring for one patient at a time.

Responsibilities

• Collaborates with patients, social workers, physicians, other practitioners, caregivers, and community agencies to coordinate and oversee the interdisciplinary plan of care.
• Affirms the health of patients dwelling in their own homes by assessing, planning, facilitating, and evaluating timely coordination of high-quality, cost-effective care.
• Improves the health of patients by promoting safety, cost efficiency, and a commitment to quality improvement.

Requirements

• A minimum of an Associate’s Degree in Nursing, with a Bachelor's Degree in Nursing preferred.
• A current State of Wisconsin or eNLC compact state RN license.
• A valid State of Wisconsin driver’s license with an acceptable driving record.
• A current BLS (CPR) certification or the ability to obtain one upon hire.
• Clinical experience preferred.
• EPIC software experience preferred.

Qualifications

• Comfortable working independently and enjoying teaching.
• A critical thinker with strong communication skills.
• Experience with pre-acute and post-acute care as well as community resources is preferred.

Skills

• Negotiation skills.
• Strong analytical and computer skills.

Pay

$20.00/hour

Schedule

M-F 0800 to 1630. No weekends. Two holidays per year- one major, one minor.

Back up on call rotation

Approximately 4 times per month with occasional on-call coverage to cover night RN PTO.

Sign on Bonus

$10k Sign on Bonus with a 2-year commitment available.
