Responsibilities

• Follow a designated care plan in accordance with the patient’s needs
• Make home visits to clients in designated geographic territories
• Administer medication, wound care, treatments, and procedures as assigned
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager
• Execute and properly document doctors’ orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations using a tablet‑based electronic system

Qualifications

• Current Registered Nurse license in Pennsylvania
• Minimum one year of recent, verifiable clinical (medical/surgical) experience
• Prior home‑care experience preferred but not required
• Graduation from an accredited nursing program (transcript or diploma required)
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; experience with electronic medical records (EMR) preferred
• Ability to travel to assigned cases

Pay

Base Pay, depending upon qualifications and subject to negotiation: $55.00 – $65.00 per hour. $5,000 sign‑on bonus for full‑time hires.
