We are seeking a full-time RN Clinical Case Manager who can demonstrate proficiency in completing OASIS and have at least one year of home health care experience. The ideal candidate should possess a positive attitude, strong skills, and a warm heart.

Requirements

Must hold a Bachelor's degree in Nursing and be licensed as an RN. One year of home health care experience and proficiency in completing OASIS are preferred.

Qualifications

Self-driven, proficient, and hardworking RNs are encouraged to apply. Must be available to work Monday through Friday with occasional weekend on-call rotations.

Skills

Proficiency in OASIS, excellent communication skills, and the ability to build trust and rapport with clients are essential.
