RN Home Health Case Manager

Responsibilities

• Assess and evaluate patient needs to develop and implement individualized care plans
• Coordinate and oversee the delivery of patient care in a home setting
• Serve as a primary contact for patients and their families regarding care and treatment
• Collaborate with healthcare teams to ensure comprehensive patient care
• Monitor and document patient progress and outcomes in compliance with policies
• Conduct patient education regarding their health, care plans, and disease management
• Manage clinical documentation and ensure adherence to regulatory standards

Requirements

• Current RN license in the state of practice
• Minimum of 2 years nursing experience, preferably in home health or community health
• Strong assessment, critical thinking, and clinical skills
• Excellent communication skills both verbal and written
• Ability to work independently and as part of a multidisciplinary team
• Familiarity with electronic medical records (EMR) systems
• Compassionate and patient-centered approach to care
