About the role
The Registered Nurse (RN) - Home Health position at Pride-Health is seeking a compassionate and clinically skilled professional to provide high-quality home health care services throughout King County and Snohomish County, Washington.
Responsibilities
- Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and other healthcare professionals.
- Provide direct and indirect supervision of Home Health Aides and Licensed Practical/Vocational Nurses in accordance with state and federal regulations.
- Conduct and document required supervisory visits for assigned support staff.
- Evaluate patient progress and adjust care plans as needed to achieve optimal outcomes.
- Deliver high-quality nursing care within the RN scope of practice and in compliance with infection prevention and control standards.
- Complete comprehensive patient assessments, including OASIS assessments and medication reconciliation.
- Maintain accurate, timely, and thorough documentation of all patient encounters in accordance with agency policies and payer requirements.
- Carefully coordinate care effectively with the interdisciplinary team to promote continuity and quality of care.
- Conduct initial and comprehensive nursing evaluations to determine home health eligibility and medical necessity.
- Communicate patient status changes, care needs, visit scheduling updates, orders, and OASIS data accurately and promptly with agency staff.
- Collaborate with physicians, patients, caregivers, and family members to ensure high-quality care and service excellence.
- Participate in performance improvement initiatives focused on reducing avoidable hospitalizations, improving patient safety, and enhancing patient outcomes.
Requirements
- Active, unencumbered Registered Nurse (RN) license in Washington State or a Multi-State Compact RN License.
- Current BLS (Basic Life Support) Certification.
- Valid driver's license, reliable transportation, and ability to travel throughout the assigned coverage area.
- Strong clinical assessment, critical thinking, and patient care coordination skills.
- Ability to work independently while managing multiple priorities in a home health environment.
- Excellent verbal and written communication skills.
Qualifications
- Previous Home Health nursing experience.
- Experience with Homecare Homebase (HCHB) and/or PointCare.
- OASIS documentation experience.
- Experience coordinating care across interdisciplinary teams.
- Strong time management and organizational skills.

Pay
$51.00 - $53.00 per hour.
Schedule
Monday through Friday, 8:00 AM - 4:30 PM. On-call requirement: Yes. Potential availability on observed holidays as needed to support patient care. Productivity expectation: 30 points per week. Mileage reimbursement: Yes, reimbursed between patient visits.
