Position Overview

The Registered Nurse (RN) in Home Health is responsible for providing skilled nursing care to patients in their homes, with a strong focus on OASIS assessments, start of care, recertifications, and discharge planning. This role supports patient independence while ensuring high quality outcomes through clinical excellence, accurate documentation, and effective communication.

Schedule & Work Details

  • Coverage Areas: Marion, Hamilton, and Johnson counties
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • On Call Requirement: None
  • Pay Rate: $51 to $53 per hour
  • Mileage Reimbursement: Yes, from first patient to last patient visit
  • Dress Code: Any solid color scrubs

Key Responsibilities

  • Perform Start of Care (SOC), discharge, and recertification visits in accordance with home health standards
  • Deliver skilled nursing services including medication setup and wound care
  • Assess patient conditions and provide care in alignment with physician orders and care plans
  • Complete OASIS assessments accurately and within required timeframes
  • Maintain timely, detailed, and compliant documentation using HCHB
  • Ensure all care delivered meets regulatory, agency, and payer standards
  • Care Coordination
  • Collaborate with physicians and interdisciplinary team members to ensure coordinated care
  • Communicate effectively with patients, families, and team members regarding care plans and updates

  • Maintain a positive attitude and strong work ethic in all interactions
  • Demonstrate excellent communication skills in both verbal and written formats
  • Support team goals and contribute to a collaborative work environment

Required Qualifications

  • Active Registered Nurse (RN) license
  • Home health experience
  • Experience with HCHB (Homecare Homebase)
  • Strong clinical and assessment skills

  • Excellent communication skills
  • Positive and patient-focused attitude
  • Strong organizational and time management abilities

Pay, Benefits, and Company Information

  • Pay Rate: $51 to $53 per hour
  • Mileage Reimbursement: Yes, from first patient to last patient visit
