About the role

As a Home Health RN, you will be a clinical leader applying your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care. You will craft individualized care, develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being. You will collaborate and coordinate seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care. You will empower and educate, serving as a vital resource for patients and their families by providing crucial teaching and emotional support. You will mentor and guide, overseeing your caseload and leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team. You will innovate with technology, utilizing leading-edge tools and best practices to optimize care delivery and maintain precise documentation.

Responsibilities

• Develop, implement, and adjust comprehensive care plans that promote patient independence and well-being.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Provide teaching and emotional support to patients and their families.
• Oversee caseload and mentor LPNs, LVNs, and Home Health Aides.
• Utilize leading-edge tools and best practices to optimize care delivery and maintain documentation.

Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

$35.53 - $52.85 /hour
